This registration form collects all the important information we need to know about your child and you, as their parent or guardian. Please read through carefully and complete all required sections.

Privacy & Confidentiality Disclaimer

All information collected through this registration form is kept strictly confidential and will only be used by Funday for the purpose of providing the best possible care for your child, ensuring their safety, and supporting their needs while attending Funday programs.

This information will not be used for marketing, promotional purposes, or shared with third parties, unless required by law or necessary in an emergency situation involving your child’s health or safety.

By completing this form, you acknowledge that the information provided is accurate and may be used by Funday staff to properly care for and support your child during their time with us.

SECTION 3: Camp Registration Information & Changes

This registration form is valid for all Funday summer camp weeks and may also be used for future Funday camp programs, including PA Day camps, March Break camps, Winter Break camps, and other seasonal camp programs.

Parents/guardians do not need to complete a new registration form for each individual camp week unless any of the information provided changes.

If any information on this form changes, it is the responsibility of the parent/guardian to notify Funday in writing by email at academy@thefunday.ca with the exact changes, so we can update the existing registration on file.

SECTION 4: CONSENTS & POLICIES

1. Child Pick-Up Authorization

Please list any additional people authorized to pick up your child (besides the parent/guardian listed above).

Photo ID will be required unless prior arrangements have been made.

Funday may share private updates with parents/guardians, including photos of your child participating in activities. These updates may sometimes include other children who are participating in the same activity.

Photos will only be shared through our secure child-management platform and only with the parent/guardian of the child or children included in the photo.

4. Camp Payment, Cancellation & Refund Policy

A 50% non-refundable deposit is required to secure your child’s place in camp. The remaining balance is due 30 days before your child’s first scheduled camp week.

If your child is registered for multiple weeks, the full remaining balance for all registered weeks is due 30 days before the start of their first camp week.

Please note that refunds are not issued for missed camp days. All camp registrations are final once completed. No refunds or cancellations are permitted after registration, except for cancellations made more than 60 days prior to the start of camp, which may be eligible for a 50% partial refund.

Cancellations made within 60 days of the start of camp are non-refundable.
